# Context DMS operations

When a document looks missing, check ingest before you check the evidence store. A file that never ingested cannot have evidence.

## Checks

1. Confirm the source product recorded an acceptance.
2. Confirm ingest stored the original bytes.
3. Confirm extract wrote a versioned evidence record.

Do not re-upload the same bytes under a new identity to "fix" a gap. That splits the history.
